Date and time: Record the date and specific time of every pest control inspection or treatment
Location: Document the exact location where pests were found or treatment was applied â which room, which corner, near which equipment
Pest type: Identify and record the pest category â rodent, cockroach, ant, termite, bed bug, mosquito, fly, silverfish, spider, or other
Infestation level: Assess severity â none, light (occasional sightings), moderate (frequent sightings), or heavy (large numbers present)
Treatment method: Record the method used â physical exclusion, sticky trapping, gel baiting, residual spraying, professional fumigation, environmental cleanup, or other
Products used: If chemical products were applied, record product name, active ingredient, quantity used, and application location â retain product labels and SDS safety data sheets
Treatment results: Re-inspect 24-72 hours after treatment â assess effectiveness (completely resolved, significantly reduced, no significant change, or worsened)
Prevention recommendations: List recommended actions to prevent recurrence â seal entry points, eliminate water sources, remove food sources, clear clutter, enhance monitoring
Follow-up plan: Set the next inspection or treatment date â light infestation: monthly; moderate: bi-weekly; heavy: weekly follow-up
Signatures: Pest control operator signature plus client or responsible person signature confirming service completion â retain records for at least 3 years
